I was going through some of my old stuff in my room tonight and came across a old book that I use to get people to fill out. I think a lot of people had them... well I remember sam and I had them. I've been reading through some of the things people wrote when it came to the little survey in it and some of them are pretty funny.

I thought it'd be fun to put the questions up here and if you want to fill it out you can and see how cheesy it is.
